The obverse depicts the star of the Order with the three stars in its center. The reverse is inscribed PAR TEVIJU ("For the Fatherland") with a flaming heart below the inscription. The "875" silver hallmark is struck into the rim at 5 o'clock on the reverse. The medal is encircled in a wreath of oak leaves which leaves the bottom part of the rim uncovered.

The Order of the Three Stars is the highest decoration in Latvia. It was established in 1924 in commemoration of establishment of the independent Latvian state. Suspended by the Soviet occupation, it was reinstated in 1994 after the collapse of the Soviet Union. The order exists in five classes, with three classes of the Medal of Honor, gold, silver, and bronze, attached but awardable separately. Both the order and the medal are awarded for meritorious service to Latvia.
